Here's my take on this. The presentation and art are really plain. Some of the art is crisp, other aspects are blurry. It leads me to believe you constructed this in MS Paint, or that you at least saved this in JPG format instead of PNG format, which would prevent blurring, especially of the red tones.
There are also two objections I have with the art choices. First off, you used a new style Mario art, at least GameCube era, if not newer. Additionally, you missed an opportunity to use the sprite version of Samus on the front, instead opting for Luigi.
Last thing, the ESRB didn't didn't establish until the Super Nintendo and Genesis/Mega Drive generation, therefore, there should not be an ESRB rating on it.

#1, I agree with all that you've said except for the ESRB. On the back, it mentions that this cartridge is made nearly 30 years after the NES's prime. Therefore, it's made during the ESRB era, and the logo is appropriate.

@ tleeart Updated the box by resaving it as a PNG file and erasing a small yellow spot on the back of the box.

I suggest a different pic of mario on the back though. he doesnt match the style of samus and link.
