I was thinking Persona 4 for this one, with the slanting on the back. I had problems with the screenshots, but I think it's sorta fixed now. Printable is higher quality if you care.

Love what you did with the Persona 4 style. The screenshots aren't that bad either.

The corner of the text box should be parallel with the dark blue border for the legal information, instead of it cutting into it.

Are you sure, because that would make a stranger shape than it is now, or make extra empty space?

Well it's updated anyway, if you don't like it, I will revert back to V1 or update further.
