Heres my newest Call of Duty 4 box. Credit to Ninty for the front template. I don't know who did the back template, but I found it on Koopadasher's VGBASimpleNeeds. Also credit to gunslinger for the the text on the back. Please rate and comment, favs are appreciated.

vision style which i like the back its alright 4/5 excellent blending

#2, good question. For some reason I can't get the hang of making many other boxes. Call of Duty boxes are the ones I am most proud of (besides my earlier one back in June last year), and the only other ones I am completely satisfied with is my Sims Death Unleashed, Playstation 2 Online, and Fury of Battle (which is made up). I am partly satisfied with a few of my newly updated boxes that I didn't want to delete... but not much. Tactical Assassin 2 was a big hit also, 10,853 views last time I checked.
